University of Arkansas K-12 Professional Development Provider Information
All University of Arkansas faculty and staff wanting to provide Arkansas Department
of Education-approved Professional Development, please use the following documents
to obtain approval. Please allow 1 month for authorization.
- Information Letter - read this first
- ADE Rules Governing Professional Development
- Assurance Form — to be completed and submitted at least one month prior to event
- Suggested PD certificate (example of form that needs to be provided to each participant upon completion of event)

Database Spreadsheet for Attendees (electronic submission required 2 weeks after event) — Suggested sign-in sheets (must
document all days and times of attendance - electronic submission required 2 weeks
after event)
- Day Attendance (Excel spreadsheet)
- In-Service Teachers (PDF)
- Sign-in Sheet (Excel spreadsheet)
- Sign-in Sheet 2 (Excel spreadsheet)
- Sign-in Sheet (Word doc)
