University of Arkansas K-12 Professional Development Provider Information

All UA faculty and staff wanting to provide Arkansas Department of Education (ADE) approved Professional Development,
please use the following documents to obtain approval. Please allow 1 month for authorization.

• Information Letter - read this first
• ADE Rules Governing Professional Development
• Assurance Form - to be completed and submitted at least one month prior to event
• Suggested PD certificate (example of form that needs to be provided to each participant upon completion of event)
• Database Spreadsheet for Attendees (electronic submission required 2 weeks after event)
• Suggested SignIn sheets (must document all days and times of attendance - electronic submission required 2 weeks after event): 1 / 2 / 3 / 4 / 5
